Initial Comments:
The purpose of this communication is to process an involuntary termination (closure) of six (6) ICF/IID certified beds; effective November 8, 2023. On August 10-11, 2023; a monitoring visit was conducted which resulted in a 90-Day Termination Action. The 46-Day follow-up was conducted on September 25, 2023. At that time a 55-Day
follow-up was planned and conducted on October 4, 2023; which resulted in the
facility remaining not in compliance with the following conditions of participation:
W102 42 CFR 483.410 Governing Body and Management and W195 42 CFR
483.440 Active Treatment Services; Requirements of 42 CFR, Part 483,
Subpart I Regulations for Intermediate Care Facilities for Individuals with
Intellectual Disabilities. Our Division recommended to The PA Department of
Human Services, Office of Long-Term Living involuntary termination (closure) of
this facilities' participation in the Federal Title XIX Medicaid Program. Correspondence dated October 18, 2023, from The PA Department of Human Services; Office of Long Term Living directed and forwarded to the facility, establishes the involuntary termination (closure); effective November 8, 2023.
An involuntary termination (closure) was processed for The Peaceful Living Inc.
Jubilee House; effective November 8, 2023.
